Romania Health Insurance for Foreigners: Verify CNAS Status by Route
In Romania, a job contract, university enrolment, residence document, family relationship, or CNP does not by itself prove active public health insurance. Start with the person's actual route, complete the responsible filing or registration, and then check the official SIUI insured-status service.
This guide deliberately does not state a fixed student exemption, a September 2025 transition, a 12-month D212 period, or dependent-person payment conditions. The previously used CNAS/ANAF pages for those details did not reopen reliably enough for a current decision.
Use the route that matches the facts

The CNAS D212 clarification is narrow. It does not establish that every foreign resident, student, dependent, or person without income can use the same option. Do not generalise it beyond the income categories and statutory provisions the page names.
Verify activation, not just submission
After payroll, D212, student registration, or S1 processing, check SIUI using the person's correct identifiers. Save the result securely with the check date. If it is inactive or the effective period is wrong, take the route-specific record to the competent county health-insurance house and ask it to identify:
- the category currently recorded;
- the effective dates;
- the missing, rejected, or mismatched declaration;
- which employer, tax office, foreign institution, or health house must correct it; and
- what document will confirm the correction.
For an employee, begin with payroll's accepted D112 record. For a D212 option, begin with the declaration receipt and payment evidence. For S1, begin with the foreign document and Romanian registration. For a student or family member, begin with the county house's written category instruction rather than a third-party checklist.
Keep cross-border coordination separate
CNAS's EU/EEA/Swiss/UK coordination page distinguishes residence-based S1 coordination from temporary-stay care. The EU's social-security forms guide says an S1 is obtained from the health authority in the state of insurance and submitted in the state of residence.
An EHIC concerns necessary care during a temporary stay. Do not use it as a substitute for residence registration when S1 or Romanian coverage is required, and do not start a Romanian contribution route merely to avoid an unresolved competent-state decision.
Build the immigration file independently
IGI's EU/EEA residence guidance uses different evidence by residence basis. Employment, study, own resources, and family files are not interchangeable. For a non-EU family-reunification case, use the current IGI family-reunification guidance and distinguish the sponsor's evidence from each arriving person's later public-insurance activation.
Before filing, ask IGI which document it accepts for the route and period: an active Romanian record or certificate, registered S1, statutory no-contribution category confirmed by the health house, or a qualifying private policy. An IGI-accepted policy does not activate CNAS, and an active CNAS result does not prove every residence condition.
Final file test
The person is ready only when the file contains both:
- current public-coverage evidence for the person and period, if public coverage is claimed; and
- the insurance document accepted for the exact immigration route, if a residence application is being made.
Keep separate rows for every family member. If the underlying CNAS category cannot be verified from an accessible current authority, record the gap and obtain a written county-house answer instead of filling it with an old rule.
If the document is for a short-stay visa rather than CNAS access, switch to the Schengen insurance proof checklist instead of reusing residence evidence.
