Bulgaria Health Insurance for Foreigners: NHIF or Private Cover?
The correct Bulgarian health document depends on why you are in Bulgaria and which institution needs proof. NHIF public coverage, an EHIC, an S1-style registration and a private residence policy are not interchangeable.
Write down nationality, employment status, residence category, arrival date and family members before choosing the evidence. Then apply this category test person by person.

The NHIF international-affairs page supports the distinction between Bulgarian obligatory insurance, temporary EHIC care and longer-term coordination documents. The EU Immigration Portal's Bulgaria responsibility map identifies the Bulgarian authority that owns the residence decision. Neither source supports a universal rule that every foreigner is automatically in NHIF or always needs private cover.
When NHIF is the route
Bulgarian employment or another statutory basis can create obligatory health-insurance contributions. Ask the employer or competent revenue authority when registration starts, who pays and what document proves active status. A personal number or residence card alone is not proof of paid or active coverage.
NHIF explains that care under the public system is delivered through contracted providers and may include patient charges. After enrollment, identify a contracted general practitioner and learn how status is verified.
Temporary and coordinated EU cover
The NHIF international-affairs page explains that an EHIC supports necessary care during a temporary stay. It also describes coordination certificates used for workers, family members and pensioners residing across states.
Do not use an EHIC as a universal long-term residence certificate. If another state remains competent, obtain the correct document and complete any Bulgarian registration step. Keep the issuing institution’s decision.
When private insurance is needed
A third-country national applying for residence may need private insurance for a period before or outside NHIF. Use the live permit checklist and test the contract for:
- insured name and passport details;
- Bulgarian territory;
- start and end date;
- required medical and repatriation scope;
- limits, deductible, waiting periods and exclusions;
- insurer authorization and contact;
- dependants;
- proof of payment and required translation.
A quote or travel brochure does not prove issued cover. Nor does acceptance for immigration create NHIF rights.
Keep residence and healthcare files aligned
For the immigration file, place each official requirement beside its evidence: identity, address, means, insurance, dates and family. The related Bulgaria residence guide covers the wider permit sequence.
For actual care, keep the NHIF registration, GP choice, private insurer claim instructions or EU coordination document. An immigration certificate may be insufficient at a clinic if it does not establish the payment route.
Fix refusals by naming the gap
Ask whether the problem is issuer, scope, term, translation, public-status evidence or the wrong residence category. Correct that fact and retain the written answer. Avoid buying overlapping policies without knowing which obligation each one serves.
Manage the transition, not just the certificate
Create a coverage timeline with four columns: immigration or free-movement status, employment or study status, responsible health system, and issued proof. The key transition dates are entry, residence approval, employment start, NHIF registration, loss of foreign coordinated cover, and private-policy expiry. A gap can exist even when every document looks valid in isolation.
For an employed newcomer, obtain the employer's registration evidence and verify that the public record is active rather than relying on a payslip assumption. For an EU-coordination case, keep the EHIC, S1, or competent-institution decision tied to the covered period and purpose. For a private bridge, check outpatient and inpatient care, emergencies, deductibles, territory, cancellation, and the exact immigration wording required.
Give each dependant a separate row. A principal's employment or policy does not prove that a spouse or child is enrolled. Recheck the position when employment starts or ends, a student graduates, a family member arrives or a permit renews. The safest Bulgarian file connects one person, one legal basis, one coverage period and one verifiable issuer.
