Who Qualifies for Public Health Insurance in Germany?

You do not qualify for German statutory health insurance (GKV) simply because you live in Germany or prefer it. Entry normally follows a legal route—most often employment subject to compulsory insurance, student status, family insurance, certain public benefits or an existing basis for voluntary membership. Start with your category

Many employees below the annual compulsory-insurance limit enter GKV through payroll. The employer reports the employment and shares the applicable contributions. Employees above the limit may have a choice, but prior coverage and the precise employment start still matter.

Self-employment alone does not create automatic GKV access. A person arriving from private or foreign coverage should not assume they can choose voluntary membership without a qualifying basis.

Prepare evidence for the route you claim

For employment, provide the contract, start date, fixed remuneration, working hours and employer details. For students, add enrolment, age and prior-insurance history. For family insurance, document the relationship, household, each person’s income and other insurance. For voluntary membership, preserve every prior insurer’s start and end dates.

Avoid three common mistakes

First, do not treat a private travel policy as proof of GKV eligibility. It may cover a transition but does not establish membership.

Third, do not assume family insurance is automatic. The fund must check relationship, residence, competing coverage and income conditions for each dependant.

After the fund accepts you

Keep the membership confirmation and effective date. Give the correct fund details to payroll, check the first payslip and verify that dependants were decided separately. The health card may arrive later; ask the fund what temporary proof to use for care.

Eligibility can change with salary, employment, studies, marriage, self-employment or a move abroad. Report changes promptly rather than waiting for a retrospective correction.
